What is required in order to upload documents?: Completed Sworn Claim Affidavit & Proof of Loss Statement – Copy of a Government Issued ID (i.e. Stated ID, Drivers License, Passport) – If required, copy of Proof of Purchase for the claimed device (i.e. customer receipt, UPC code form a PO Box) – If required, copy of supporting documents (i.e. insurance policy, court documents, police report). The documents can be digital pictures or scanned for upload. AT&T document upload also allows the customer to fax in the documents and AT&T will upload them (this is convenient service but the documents will take longer to upload). Any questions in regards to the document upload process can be directed to 1-888-562-8662 and an Asurion Customer Service will be MORE than happy to answer any questions or concerns the customer may have. The toll-free help line phone number is open from 7 am and 9 pm CST Monday thru Friday or Saturday and Sunday between 8 am and 8 pm CST.
